Register with the surgery

Catchment area

To check whether you are eligible to register with the practice, please enter your postcode into our catchment area checker.

If you live just outside St Mary’s Surgery’s catchment area, please contact us. We may still be able to accept your registration, subject to our current patient capacity.

Paper copies are also available from the surgery reception.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

When returning the form to the surgery, please provide photo ID and evidence of your address such as a utility bill or bank statement.

Newborn babies

Please bring your copy of the mother and baby discharge summary to the surgery so your baby can be registered. We would also request that you bring their ‘red book’ to their first appointment. Once the surgery has received your discharge summary, your baby will be registered with the surgery.
