If you would like to request a medical report, please use this form. You will need to call the surgery to book an appointment and pay any fees relating to your request.

Please be aware there will be a charge of between £50 to £130 for reports. Please contact the surgery to confirm the charge before requesting.

For adoption and fostering, please bring the relevant forms into the surgery to discuss.

To complete a Subject Access Request, please fill out a Subject Access Request (SAR) form online or in the surgery.
