Patients who live within the practice boundary are welcome to apply to register with the Practice. You may see the Doctor of your choice if they are available, but you should be prepared to see any Doctor in an Emergency.

The Practice Area is South of Weyhill Road, to Weyhill, Monxton, Abbotts Ann, Goodworth Clatford, Wherwell, up to the A303, to A3093 Churchill Way, along Northern Avenue, Western Avenue, Western Road back to Weyhill Road.

Follow the signs for the Iron Age Museum from anywhere in Andover, you will find us in the same road, Church Close, on the left before you arrive at the museum. We provide access and washroom facilities for Disabled Patients.

Registering with the Practice

If you are within our catchment area and wish to register with our Practice then please Register Online or print the forms below and bring them into the Surgery along with photo identification and separate address verification.

Please ensure that all relevant forms are completed in full, any forms with missing information will be rejected and we will be unable to register you.

Once we have verified you and checked that your forms are completed in full then we will register you on our system. Please note that this may take up to 48 hours.
