Medical report request

Use this service to request a medical report. You will need to call the surgery to book an appointment and pay any fees relating to your request.

Please be aware there will be a charge of between £50 to £130 for reports. Please contact the surgery to confirm the charge before requesting.

You can use this service if you:

  • are registered at the surgery

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

For adoption and fostering, please bring the relevant forms into the surgery to discuss.

To complete a subject access request, please fill out a subject access request (SAR) form online or in the surgery.
